Do I have the option to do not install the back board, so I have a regular bed look?
No, unless you plan to create some other kind of back to keep the bed stable. The trundle slats would not have any support on the back side. If you handy you can create another board to replace the back board. It would be something similar to the front support which is only about 4 to 6 inches wide.

Does the twin bed and trundle both fit twin size mattresses?
Both the trundle and the bed uses a regular twin size mattress. I bought the durable all foam ones so If one sit on the side like a sofa you wouldn't feel any springs that are built in the normal mattresses.
